Requirements for All Student Athletes

All students wanting to participate in school sports are required to have:

  1. Middle School Activities Participation Form
  2. PCS Student Accident Insurance (even if you already have family health insurance)
  3. Physical from your doctor
All three documents must be submitted to the school TOGETHER as a completed packet. Individual forms will not be accepted.
